Member Cancellation

Your Wellness Program membership is auto-renewable. Following the initial term, your Wellness Program membership will automatically continue on a month-to-month basis at the above agreed upon cost per month until your Wellness Program membership is cancelled using the appropriate process starting with completing the form below.

Upon approval of your written request for Wellness Program membership cancellation during the initial membership term, you will be relieved from making any future Wellness Program membership dues payments. All Wellness Program membership cancellations require 30 days written notice and are effective 30 days after the date received. Payments due prior to the effective date will be charged as scheduled.

You may cancel your Wellness Program membership during the initial term only in the event that: (a) you permanently relocate your residence more than a 25 mile radius away from any Vida-Flo; or (b) a physician certifies that you are unable to receive Vida- Flo services. All Wellness Program membership cancellation requests MUST be accompanied by written proof of relocation (e.g., Mortgage or Rental agreement, utility bill, car insurance) or submission of Licensed Medical Doctor’s (M.D.) note.
