What IV Saline Does for POTS, and When It Is Not the Answer

Postural Orthostatic Tachycardia Syndrome (POTS) is a condition characterized by an abnormal heart rate increase after sitting up or standing. It is a form of dysautonomia, an autonomic nervous system disorder, which controls many involuntary body functions such as heart rate, blood pressure, and digestion.

Expanding blood volume is a real part of POTS management, and IV saline is one way to do it. The Heart Rhythm Society expert consensus statement on POTS says it is reasonable to treat a short-term decompensation with an acute infusion of up to 2 liters of saline, and reports that 1 liter over about an hour can reduce orthostatic tachycardia and ease symptoms for several hours, sometimes up to two days. The same statement is equally clear that regular infusions are not recommended and that chronic or repeated intravenous cannulation is potentially harmful. So we will treat a bad day. One of the main symptoms of POTS is orthostatic intolerance, the inability to tolerate standing for long periods. Low blood volume is part of the picture for many people with POTS, and that is what an infusion addresses, temporarily. Here is what that does and does not mean.

  • Chest pain: Discomfort or pain in the chest.
  • Brain fog: Difficulty concentrating and memory issues.

The exact causes of POTS are not fully understood, but it is believed to be linked to a variety of factors, including:

  • Genetics: A family history of POTS or other forms of dysautonomia.
  • Automimmune diseases: Conditions like lupus or Sjögren’s syndrome.
  • Infections: Viral or bacterial infections that affect the autonomic nervous system.
  • Trauma: Physical injury or severe emotional stress.
  • Hormonal changes: Such as those occurring during puberty, pregnancy, or menopause.

While there is currently no cure for POTS, several treatments can help manage the symptoms and improve quality of life. These include:

  • Lifestyle changes: Increasing salt and fluid intake, wearing compression garments, and engaging in regular, gentle exercise.

There is no cure for POTS, and a drip is not one. What an infusion of saline can do is expand blood volume and take the edge off a short-term decompensation, which the Heart Rhythm Society expert consensus statement considers a reasonable thing to do with up to 2 liters. IV therapy works by using an intravenous (IV) fluid drip to deliver saline solution through a small catheter and tubing directly into the bloodstream. This saline solution may also include medications and vitamin supplements tailored to the individual patient.

During your therapy session, a medical professional will insert an IV line attached to a bag of saline solution. This bag is hung above the patient's head, using gravity to regulate the flow of fluids into the bloodstream. Compared to a syringe or other forced technique, gravity allows for a gradual introduction of fluids.

While the time frame of a single treatment varies from person to person, a session typically lasts 30 – 45 minutes. There are exceptions to this timeframe, such as with our NAD IV treatments, which generally take 1 to 2 hours per session.
